How do I explain equivalent fractions simply?

Same amount, different sized pieces. Half a chapati and two quarters of the same chapati are the same food - that comparison usually settles it.

Why did my child write 1/4 + 1/4 = 2/8?

They added the denominators too. The denominator says how big each piece is, so it stays the same; only the count of pieces is added, giving 2/4.
