Must my child memorise all the tables before this worksheet?

Not all of them. Keep a table chart to hand for the facts that are still shaky, so the worksheet can build the column method rather than stalling on recall.

Why is my child good at tables but wrong on long multiplication?

Almost always carrying and column alignment. Use squared paper or draw the columns, and circle every carried digit - this fixes the majority of these errors.

Should the worksheet be timed?

Time the fact recall if you like, but leave the multi-digit questions untimed. Speed there causes exactly the slips you are trying to remove.
