Do we need base-ten blocks for this worksheet?

No. The worksheet is complete with pencil and paper. Any ten identical small objects plus an elastic band do the same job as shop-bought blocks.

How high should Grade 1 place value go?

Tens and ones up to 100 is the standard Grade 1 range. Hundreds are usually a Grade 2 skill, so there is no need to push past 100 before two-digit work is comfortable.

Is expanded form (30 + 6) really necessary this early?

It is worth doing because it makes the value of each digit visible, and it is the exact idea your child will lean on for two-digit addition next year. Keep it short - a few numbers per session is plenty.
